Employment forms for student coordinators
  • Employment Information Form [download PDF]
    All Student Coordinators must complete and submit one of these forms to the CAS office (either in person or electronically) at the beginning of the academic year. Delays in submitting this paperwork will result in delays in being paid. NOTE: If any of your information changes during the course of the year, especially if your payroll status changes (You get a new job or cease working at a previous job) you must contact the CAS office with the new information immediately.


  • IRS Form W-4: Employee's Withholding Allowance Certificate [download PDF]
    Student Coordinators must return this form, completed and signed in ink to the CAS office.


  • State Form IL-W-4: Employee's Illinois Witholding Allowance Certificate [download PDF]
    Student Coordinators must return this form, completed and signed in ink to the CAS office.


  • IRS Form I-9: Employment Eligibility Verification Form [download PDF]
    Student Coordinators must return this form, completed and signed in ink, together with the necessary identification (see I-9 form instruction sheet), to the CAS office if they have never been on the University payroll.


  • University of Chicago Student Casual Payment Request Form [download PDF]
    Student Coordinators who are not employed elsewhere in the University must submit this form each quarter to be paid. Please fill out only the "Personal Data" section (top three lines) and sign in the appropriate space. Then drop off the form in the CAS office to be completed and submitted to payroll. We cannot accept faxed or reproduced signatures.


  • University of Chicago Biweekly Additional Payment Request Form [not available online]
    Student Coordinators who are employed in the University and paid on a biweekly basis must come to the CAS office to fill out and sign this form each quarter to be paid.


  • University of Chicago Monthly Extra Service Payment Request Form [not available online]
    Student Coordinators who are employed in the University and paid on a monthly basis do not need to come to the CAS office, however they must e-mail the CAS office to confirm their payroll status each quarter.

American guest speakers
  • Speaker's Fee Statement [download PDF]
    This form states the date, name of the speaker, paper given, workshop name(s) and agreed upon honorarium amount (Note: The Council on Advanced Studies will contribute a maximum of $150 honorarium per speaker per workshop.) This form must include the speaker's signature, home address, and Social Security number.


  • IRS Form W-9 [download PDF]
    Request for Taxpayer Identification Number must be completed and signed.

International guest speakers

Please note: Depending on the type of visa the visitor has, additional forms may be needed. Please consult the University's checklist of required forms according to visa-type. For further help, contact the CAS office with any questions.

  • IRS Form W8BEN-I: Certificate of Foreign Status of Beneficial Owner for U.S. Tax Withholding [download PDF]
    This form is required of all visiting speakers receiving an honorarium who are not citizens of the United States.


  • UPP 192: Alien Determination of Residency Form [download PDF]
    All speakers who are not U.S. citizens must fill out this form.


  • IRS Form 8233: Exemption of Withholding of Income Tax [download PDF]
    Speakers who are not residents of the United States may wish to claim exemption from their taxes. Speakers who do not complete this form will have taxes withheld at a rate of 30% (an honorarium of $150 would be reduced to $105). To qualify for tax exemption, the United States must have a tax treaty with the speaker's country of residence.

Travel reimbursement
  • Special Event Profile [go to online form]
    The Special Event Profile form is used to make advance reservations with American Express for guests travelling to Chicago to participate in a workshop.

  • The Travel Expense Voucher [not available online]
    This form must be picked up at the CAS office in Judd 443. If the office is closed, forms can be found in the folders to the right of the door. Please make sure you have all receipts and a signature before submitting the form.

Expense reimbursement

    This form is to be completed and attached to receipts submitted for reimbursement.
